In 2026, industry benchmarks put the median monthly retainer for a two-day-per-week engagement at approximately $15,000, with a broader range of $8,000 to $25,000 monthly depending on cadence and industry premium.7 Life sciences engagements typically sit at the upper end of that range because the required regulatory, quality, and platform expertise is scarcer than SaaS-generalist expertise.
Cost framing for the board. A full-time biotech CTO carries a fully loaded cost of $300,000 to $500,000 including base, bonus, equity, benefits, and recruiter fees. A fractional CTO at $15,000 to $22,000 per month across an 18-month engagement runs $270,000 to $396,000 total, which corresponds to roughly 30 to 50 percent of the cumulative full-time cost over the same window while the organization matures toward a permanent hire.
Transitional-to-Full-Time Paths
A fractional CTO is often the on-ramp for a full-time hire rather than a permanent operating model. Three transitional paths are common in biotech.
Bridge-and-recruit
The fractional CTO defines the target-state architecture, builds the roadmap and organizational structure, and then partners with the board on the search and hiring process for a permanent CTO. Typical bridge duration: 9 to 18 months.
Fractional-to-full-time conversion

The first 90 days set the tone for the entire engagement. In our practice, the pattern that works is a three-phase cadence: 30 days of technical due diligence, 60 days to secure meaningful quick wins alongside the diligence findings, and 90 days to deliver a formal technology strategy the board can vote on. This structure mirrors the disciplined post-acquisition playbooks used in pharma M&A integration, where technical misalignment is the single largest cause of integration failure.8
Days 0-30: Technical Due Diligence
The first 30 days should be almost entirely listening and inspecting. The output is a diagnostic that the CTO can defend to any auditor, board member, or successor.
The Diligence Inventory
- Systems inventory: Every application in use across R&D, Quality, Manufacturing, Clinical, Regulatory, Commercial, Finance, and HR, mapped to owners, business criticality, GxP scope, and validation state.
- Data assets and flows: Where scientific and regulatory data lives, who owns it, how it moves between systems, and where the pipelines are brittle or undocumented.
- Cybersecurity posture: Identity and access management maturity, endpoint protection coverage, backup and recovery tested state, third-party risk program, and OT/manufacturing exposure.
- Vendor and contract map: All active technology vendors, contract renewal dates, spend, and consolidation opportunities.
- Team and organizational structure: Headcount, skills, reporting lines, open roles, and the effective decision rights across IT, Quality, and business functions.
- Open regulatory findings: Any 483 observations, warning letters, EMA GMP inspection findings, or internal audit observations that touch computerized systems.
Diligence red flags to name explicitly. Unvalidated GxP systems in production, offline or manual “shadow” ELN and LIMS workarounds, expired backup restoration tests, orphaned admin accounts, vendors past end-of-support, and undocumented pipelines that only one person understands. Each of these becomes an inspection finding or an incident, and each is best surfaced in week two rather than month twelve.
Days 30-60: Quick Wins
Quick wins are the currency of trust. A fractional CTO who spends 90 days in diligence with no visible progress loses the room. The right quick wins are decisions the organization has been avoiding, low-risk cleanups that produce visible relief, and small structural changes that pay compounding dividends. Examples that consistently work in biotech include the following.
- Retire two or three shelfware systems and cancel the associated renewals; this typically funds a chunk of the CTO’s own retainer within the first quarter.
- Establish a weekly IT-Quality standing meeting with a shared risk register, replacing ad hoc coordination.
- Kill an unvalidated shadow system and replace it with the validated path, on a compressed timeline the CTO personally sponsors.
- Consolidate identity and access management under a single directory with MFA enforced across all GxP and financial systems.
- Stand up a formal change advisory process for GxP-relevant changes, replacing the tribal-knowledge model.
Days 60-90: Strategy Formulation
The 90-day deliverable is a formal technology strategy: a one-page vision, a 12- to 18-month roadmap with major milestones and dependencies, a target-state architecture diagram, a cybersecurity risk statement, and a resource plan showing internal roles, vendor spend, and any additional fractional or contract help required. The strategy should be defensible to the board, the auditors, and the future full-time CTO, and it should be the anchor document that gets updated quarterly for the remainder of the engagement.9
What good looks like at day 90. The CEO can describe the technology strategy in two minutes. The Head of Quality can point to a standing operating rhythm with IT. Trigger Events That Signal Time to Transition
- Funding milestone: Closing a Series C or later that adds a permanent technology executive to the org chart.
- Manufacturing milestone: Standing up a first GMP manufacturing site or CMO relationship that requires resident, full-time technology leadership on-site.
- Regulatory milestone: Filing an IND, CTA, or first BLA that shifts the technology stakes and inspection profile.
- Team size: Reaching an internal IT and technology team that exceeds roughly 12 to 15 people, at which point the management burden alone justifies a full-time hire.
- Commercial launch: First commercial launch, which typically pulls the technology function into daily operational tempo that fractional models cannot serve.
The Six-Month Transition Playbook
Month 1: Confirm the trigger and align the board
The fractional CTO presents to the board a formal recommendation to transition, including the specific trigger event, the profile of the target full-time CTO, the compensation range, and the estimated timeline. The board approves the search.
Month 2: Job description and search firm engagement
Write the JD anchored in the six-domain scope, not a generic template. Engage a life-sciences-focused executive search firm. The fractional CTO participates in candidate calibration.
Month 3-4: Candidate interviews and offer

Month 5: Structured knowledge transfer

What to Hand Off in Writing
The written handoff artifact is a “CTO Operating Book” that includes the current roadmap, the architecture diagrams, the vendor register with renewal dates, the risk register, the org chart with retention notes, and a candid annotated list of open decisions and their history. This document is also the artifact that would let the board recover cleanly if a new hire did not work out. It is worth writing well.
The Sustained-Fractional Alternative
Not every biotech should transition to a full-time CTO. For platform-oriented biotechs, holding companies, and portfolio companies of a strategic investor, a sustained fractional model can outperform a permanent hire well past the 24-month mark. The signals that a sustained model is the right choice include a stable IT and technology headcount below 15, a manufacturing footprint that is fully outsourced to CMOs and CDMOs, a research operating model that leans heavily on external computational and analytics partners, and a board that values continuity of senior judgment over dedicated executive-team presence. In this scenario, the engagement letter should shift emphasis from “transition plan” to “annual renewal review” and add explicit language on knowledge continuity, backup coverage during the CTO’s other engagements, and IP handling across multiple client relationships.
Common Pitfalls Boards and Founders Should Avoid
A fractional CTO engagement can go wrong in ways that are almost always visible in the first 60 days. The following patterns come up in every conversation we have with boards and founders considering this model.
Buying advice instead of accountability. A fractional CTO is not a consultant delivering slides. They own outcomes. If the scope reads like a series of workshops, it is under-scoped. The retainer should buy senior judgment applied to real decisions with real accountability, including named quarterly deliverables.12
Under-weighting the IT-Quality partnership. A CTO who cannot fluently discuss computer system validation, 21 CFR Part 11, Annex 11, and computer software assurance is the wrong CTO for a biotech, regardless of otherwise impressive credentials. Test for this in interviews with a specific system-scenario question, not a general “tell me about your GxP experience.”
Ignoring cybersecurity. Biotech and pharma organizations sit in the top tier of ransomware and data-exfiltration targets, with recent public incidents at West Pharmaceutical Services in May 2026 and Inotiv in August 2025.1314 If the fractional CTO scope does not explicitly include cyber oversight, the board is buying a plaintiff-favorable governance gap.
Blurring lines with a vCIO or interim CTO. A fractional CTO differs from an interim CTO (short-term full-time gap-filler) and a virtual CIO (typically more IT-operations focused). Boards should be explicit about which role they are buying and adjust the scope, cadence, and cost accordingly.15
Skipping the transition plan. Boards frequently defer the transition-to-full-time conversation until it becomes urgent. Better to name the transition triggers on day one, in the engagement letter, and revisit them each quarter. That prevents the awkward moment when the fractional CTO becomes a bottleneck and no one has planned the exit.
