Your CAPA Closed.
The Problem Didn’t.

Recurring Deviations Are Rarely a Training Problem

Most sites investigate the same failure more than once a year. The investigation closes, the CAPA is approved, and a few months later it happens again. When we look at why, the answer is almost never that people stopped following the procedure. It is that the process makes the error easy to make and hard to catch, and the original investigation stopped at the first plausible cause instead of the real one.

That distinction matters more in a regulated plant than almost anywhere else, because a fix that does not hold becomes an observation the next time an inspector reads through your deviation history.

How We Engage

Three Ways to Work With Us

How much support you need depends on whether you already know where the problem is.

Tier 1

Process Assessment and Problem Diagnosis

We map the process as it actually runs, review your deviation and CAPA history, and show you where the recurring losses are coming from. You get a prioritized findings report with the specific actions we would take first, and the reasoning behind each one.

Best when: You know something is wrong, but not where.

Tier 2

Targeted Improvement Projects

We lead one improvement through to completion, using DMAIC, 8D, or A3 depending on what the problem calls for. We stay through the effectiveness check, so you finish with evidence the fix worked rather than a signature saying it should have.

Best when: You know the problem and need it solved properly.

Tier 3

Embedded Operational Support

We deliver improvements alongside your team and teach the methods as we go, so the capability stays with you after the engagement ends. Sites choose this when they want to stop bringing consultants back for the same category of problem.

Best when: You want the capability, not just the outcome.

Methodology

The Methods, and When Each One Fits

These are not interchangeable, and choosing the wrong one is a common reason improvement projects stall.

  • A3 Problem Solving: Works a problem to root cause on a single page, which forces clarity and keeps the thinking reviewable
  • 8D Root Cause Analysis: Contains the problem first, then finds the cause and proves the fix held. The method of choice when something is actively going wrong
  • DMAIC: Improves an existing process without breaking what already works, using data rather than opinion
  • FMEA: Identifies how a process can fail before it does, and ranks what to address first
  • PDCA: Turns a good idea into a verified change, and stops improvements from reverting once attention moves elsewhere

Specialist Depth

Depth in Sterile and Aseptic Operations

Sterile manufacturing is where process problems carry the highest consequence, and it is where our deepest experience is.

  • Aseptic processing and contamination control strategy
  • Media fills and aseptic process simulation
  • Environmental monitoring program design
  • Visual inspection and inspector qualification
  • Cleaning validation
  • Aseptic batch release
  • MHRA Annex 1 gap closure for existing sterile operations

Improvements That Survive an Inspection

Any consultancy can make a process faster. In a GMP environment the harder requirement is that the change is justified, documented, and defensible when someone from the FDA or EMA asks why you made it.

We prepare teams for both US and EU inspections, including front room and back room practice, and we build that same standard of evidence into the improvement itself, so you are not reconstructing the rationale two years later.

Why Sakara Digital

Lean Expertise and GMP Reality, From the Same Team

Most process improvement consultancies have never worked inside a GMP environment, and most GMP consultancies do not do process improvement. You usually end up choosing between someone who knows Lean and someone who knows what an inspector will ask.

We bring both, and the digital side comes with us. When the right answer to a recurring deviation turns out to be a system change rather than a procedure change, that is something we can take on as well.

Let’s Find Out Why It Keeps Happening

Tell us about the failure you keep investigating. We will tell you whether it looks like a process problem, a system problem, or something else, and what we would do about it.
