Why the Fractional Regulatory Ops Model Emerged Now
Three forces converged over the last twenty-four months to make fractional regulatory operations a mainstream option rather than a curiosity. The first is the funding environment. Biotech capital moved carefully through 2024 and 2025, and while 2026 is bringing what industry analysts describe as a “genuine thaw,” sponsors remain disciplined about permanent headcount decisions.2 A late-stage biotech with a single lead asset in Phase 3 does not want to add a $400,000 base plus equity plus benefits before the product’s commercial reality is proven, and the board does not want to see that line either.
The second force is the widening acceptance of fractional talent as a legitimate operating model rather than a stopgap. Deloitte’s 2025 life sciences outlook reported that nearly 60 percent of surveyed executives identified optimizing the operating model as a priority, with rethinking of workforce structures explicitly on the agenda.3 Fractional engagements have moved from “we could not find a full-time hire fast enough” to “we deliberately want fractional because it fits the workload.” That shift matters. It means fractional leaders no longer have to explain their model; boards understand it.
The third force is the maturation of the regulatory outsourcing ecosystem. Contract research organizations, dedicated publishing vendors, and regulatory information management platforms now cover the tactical execution layer with real depth. Fifty-four percent of biotech and pharma companies outsource regulatory activities, and among emerging biopharma the number climbs to 62 percent; pharmacovigilance sits even higher.1 With execution capacity available on demand, the question shifts from “who does the work” to “who owns the strategy and coordinates the vendors.” That is the fractional leader’s job, and it does not require forty hours a week to do it well.
A fourth quieter force also deserves mention: the population of experienced regulatory operations executives willing to accept fractional roles has grown. Post-merger consolidation, biotech restructurings, and the ordinary tempo of pipeline decisions have produced a generation of senior operators, many of them former VPs of regulatory affairs at now-acquired biotechs, who have chosen portfolios of two or three fractional engagements over another full-time role. Fifteen years ago that population barely existed. Today it is the primary talent pool for the model. That supply-side reality is why sponsors can actually find qualified fractional leaders now when a decade ago the search would have taken longer than a full-time recruit.
Put together, these four forces explain why the model has moved from experimental to normal in mid-cap biotech regulatory operations. Cost pressure creates the demand. Board acceptance clears the political path. Vendor maturity makes the delivery model viable. And practitioner supply makes the hires possible. When any one of those four is missing the fractional model gets harder; when all four align, as they now do, it becomes the default answer for a specific band of biotech operating profiles.
What a Fractional Regulatory Ops Leader Actually Does
The confusion around fractional regulatory ops leaders usually stems from a scoping problem. The role is not a part-time regulatory strategist, not a part-time submissions publisher, and not a part-time regulatory affairs director. It is a specific coordination and accountability function that sits above execution vendors and below the head of regulatory affairs. When mid-cap biotech leaders describe the role to us in a way that ends up working, it looks like this.
The fractional regulatory ops leader owns the submission plan. That means the master timeline for the current lead application across every region in scope, the module-by-module content plan, the vendor RACI, the risk log, and the health authority interaction calendar. They do not personally write module three CMC sections and they do not personally publish the eCTD sequence, but they own the plan that says who will and by when. In practice this is a Monday and Wednesday commitment for most engagements, plus availability for the Tuesday leadership standup and a Friday risk review.
They own the vendor stack. Regulatory operations at a Series C biotech typically involves at least a CRO for clinical documents, a publishing partner (Certara, Cardinal Health, Freyr, PharmaLex, or similar) for eCTD assembly and validation, a regulatory information management (RIM) platform, and a translation vendor for non-English submissions.67 Each of those relationships needs a governance rhythm and a decision owner. The fractional leader runs the operating cadence with each vendor rather than delegating vendor management to a coordinator who does not have the authority to escalate.
They own the health authority correspondence log. Every question from FDA, EMA, MHRA, Health Canada, PMDA, or TGA lands in a single tracked queue with an owner, due date, and dependency chain. The fractional leader does not draft every response, but they set the standard for what a response looks like, they approve it before it goes out, and they own the metrics on responsiveness.
They own the readiness assessments. Before every major milestone (pre-NDA meeting, Type C meeting, filing, response to a complete response letter, marketing authorisation validation) the fractional leader runs a readiness assessment against the sponsor’s own quality standard. This is where a fractional leader earns their fee, because they have almost always seen the same readiness patterns at three or four prior sponsors and can tell you which gap is real and which one is theatre.
The scope test. If you can describe the role as “own the plan, own the vendors, own the correspondence, own the readiness,” and the person you hire can operate at that level of ownership without hand-holding, you have a fractional regulatory ops leader. If the role degrades into “help us figure out what to do next,” you have a consultant, and the pricing model should reflect that.
What the role is not
A fractional regulatory ops leader is not the sponsor’s regulatory strategy voice. Strategy sits with a head of regulatory affairs, a chief medical officer, or a VP of clinical development who owns the therapeutic and evidentiary story. The ops leader owns the execution of that strategy against a submission timeline. When those two roles blur, the engagement usually fails because either strategy calls take too long to work through the fractional cadence, or the ops function stalls waiting for strategy inputs. Keep them separate.
The role is also not a substitute for a submissions publisher. Publishing is a specialised discipline that requires deep familiarity with eCTD granularity rules, hyperlinking conventions, module 1 regional variance, and platform-specific tooling in Lorenz, EXTEDO, or Veeva Vault Submissions.6 The fractional leader manages the publishing vendor. They do not do the publishing.
When Fractional Fits and When It Does Not
The single most important question a mid-cap biotech leadership team can ask before hiring in this space is not “should we go fractional or full time?” It is “what is the sustainable workload of the role two years from now?” If the answer is “we will have three commercial products across five regions with active lifecycle management,” you should be planning for a full-time VP of regulatory ops today and using a fractional bridge until you close the search. If the answer is “one product, one region, and maybe a second region in year three,” you almost certainly have a permanent fractional workload, not a bridge.
Series C, first NDA or MAA in prep
Submission cadence is high but concentrated. Ops leader coordinates the CRO and publishing vendor through filing, then throttles down to lifecycle management. Fractional fits the workload shape.
Expansion to a second region
US-approved product now going to EU or vice versa. Ops leader owns the region-specific Module 1 plan, health authority relationships, and vendor add-ons without needing a permanent second FTE.
Post-merger integration
Two regulatory ops functions merging into one. Fractional interim leader stabilises the combined portfolio while the permanent structure is designed.
Between permanent leaders

If FDA or EMA has issued a warning letter or a critical inspection finding, sponsor accountability demands a full-time senior in the seat. Fractional visibility is insufficient.
The other classic misfit is a biotech that treats fractional as a way to avoid making an organisational decision. If the board has a real ambition to be a commercial-stage multi-product company within five years and is planning three simultaneous global filings, the fractional leader can bridge for a while, but they should be running an active search alongside the engagement. When we see a fractional arrangement drift into year three without a permanent plan, it is almost always because the sponsor never made the decision, and the fractional leader ends up carrying more than the model was designed to carry. That is a failure mode.
Fractional-Plus-CRO Layering
The fractional model does not replace the CRO. It layers on top of it, and understanding that layering is essential to making the economics work. A CRO provides tactical capacity: submission specialists, regulatory writers, publishing engineers, and quality reviewers who execute the modules and assemble the sequences. Some CROs also offer strategic regulatory consulting, and for early-stage sponsors that can be sufficient. As soon as the sponsor is orchestrating multiple vendors across multiple regions, however, the CRO is one voice among several, and the sponsor needs someone whose only allegiance is to the sponsor’s plan.
That someone is the fractional leader. The layering looks like this in a typical mid-cap engagement:

The critical design choice is whether the fractional leader chooses and manages the CRO and publishing vendor or inherits them. In most engagements we have seen work, the sponsor lets the fractional leader either endorse or replace the existing vendor stack within the first 60 days. If the leader is stuck with a publisher they would not have chosen and cannot get out of, the model tends to stall.
SD perspective. The fractional-plus-CRO stack is the model most mid-cap biotechs actually need, and yet it is the least discussed. The industry conversation tends to be “hire a VP” or “outsource to a CRO,” as if those are the only two options. The reality is that the sponsor still needs a single, accountable orchestrator whose incentives are aligned only with the sponsor, and that orchestrator does not need to be full time for most of the sponsor’s operating history.
Cost Economics Versus a Full-Time Hire
The economics conversation is where most fractional engagements are won or lost with the CFO. The pushback we hear most often is a version of “a full-time VP costs $400,000 fully loaded; if the fractional leader is $250,000, are we really saving anything after we add the CRO?” The answer is yes, but only if you compare like for like, which most biotech finance models do not.
Start with the full-time comparator. A VP of regulatory affairs in the US base is around $174,543 on average per Comparably, with San Jose reaching $344,615 and large pharma US roles starting at $400,000 base.9 Adjust for a biotech-appropriate benchmark and load in employer taxes, benefits, and equity. A realistic fully-loaded cost for a full-time VP of regulatory ops at a mid-cap biotech in Boston, San Francisco, or the DC metro area sits between $475,000 and $700,000 in year one, before you count the recruitment fee (typically 25–30 percent of base for retained executive search) and the onboarding lost productivity of the first six months.
Now the fractional comparator. A senior fractional regulatory ops leader with a proven NDA or MAA under their belt typically prices in the $250 to $400 per hour range for an ongoing retainer. Two days per week (16 hours) at $300 per hour across a 48-week working year is $230,400. Three days per week is $345,600. Add the fractional leader’s coordination overhead into the CRO and publishing scope, and the total fractional stack often lands 30 to 45 percent below the full-time comparator for a first-NDA workload.
The larger point, however, is that this comparison misses the real economics. The cost that matters is not the leader’s line-item cost. It is the cost of a three-month regulatory delay, which one major sponsor has estimated at $50 to $100 million in lost revenue for a commercial product.11 A fractional leader with three prior NDAs under their belt reduces the odds of that delay more than a first-time full-time hire does, because the pattern-match on readiness gaps is deeper. When the finance conversation reframes from “how much does the role cost” to “how much does the delay cost, and which staffing model reduces that risk most,” the fractional-plus-CRO model wins most Series C decisions.
The equity trap
One accounting nuance worth flagging: the fractional leader is a service provider, not an employee, which means the sponsor is not creating a dilutive equity grant to fill the role. For a Series C biotech where every equity point matters, avoiding a 0.1 to 0.5 percent grant to a permanent VP hire is a meaningful saving that finance rarely puts on the comparison table. Ask your CFO to add it.
The ramp-down question
Full-time hires create a downside asymmetry that fractional does not. If a lead asset fails in Phase 3, gets a complete response letter that forces a costly redesign, or gets deprioritised, the sponsor still owns the full-time hire, and a workforce reduction at the VP level carries severance, morale, and reputational costs. A fractional engagement can throttle down in ninety days with a professional off-boarding. The optionality itself has economic value, and in a portfolio-based decision analysis it should be modelled explicitly. In our experience it is worth two to three months of full-time cost when priced properly, which shifts more mid-stage decisions toward fractional than most CFOs realise.
What the finance comparison misses on the other side
To be fair, the fractional model also carries real costs the CFO comparison sometimes ignores. Onboarding a fractional leader still takes real sponsor time. Vendor renegotiation carries transition risk. And the sponsor has to invest in the internal role that partners with the fractional leader (usually a director-level regulatory ops manager) or the engagement stalls when the sponsor cannot answer basic access, records, and decision questions between the leader’s on-days. Budget those costs in. They are typically 15 to 20 percent of the leader’s fee and they are real. The point is not that fractional is always cheaper; it is that the fully-loaded comparison, when done honestly, still comes out in fractional’s favour for the workload band this article covers.
An Engagement Model Template
The engagement model is where good intentions succeed or fail. The following template is what we have seen work in mid-cap biotech engagements for a first NDA or MAA workload. Adapt to your circumstances, but do not skip the structural elements.
Weeks 1–4: Diagnostic and stabilisation
Fractional leader spends four weeks at four days per week doing the intake: submission plan review, vendor audit, RIM configuration check, health authority interaction history, risk log stand-up. Deliverable is a written diagnostic that names the top five ops risks and a proposed 90-day plan.
Weeks 5–12: Vendor endorsement or replacement
Fractional leader endorses or replaces the CRO regulatory scope, the publishing vendor, and the translation vendor. Puts written service level agreements in place with each. Establishes the operating cadence: weekly ops standup, biweekly vendor review, monthly steering.
Weeks 13–26: Steady-state ops
Cadence throttles to two-and-a-half days per week. Fractional leader runs the plan, drives readiness assessments at each milestone, owns HA correspondence quality, escalates to the head of regulatory affairs weekly. This is the settled state.
Weeks 27–52: Filing sprint
Time commitment steps back up to three days per week for the eight to twelve weeks around filing. Post-filing, throttles down to one-and-a-half days per week for the review period, ramping back up around advisory committee, pre-approval inspection, and labelling negotiation.
Post-approval: Region expansion or lifecycle
Fractional leader either drives the next region (MAA if NDA came first, or vice versa) or transitions the sponsor to a lifecycle-only cadence. Decision on whether to convert to a full-time VP is made here based on projected two-year workload.
Contract structure elements to insist on
- Named individual, not a firm rotation. The engagement value is the specific leader’s judgment and pattern recognition. Firms that reserve the right to swap the lead consultant should be declined.
- Minimum time commitment floor. Two days per week is the practical minimum for continuity; below that, cognitive overhead of re-entering the sponsor’s context eats the productive time.
- Written non-compete on competing lead assets. The leader should not be simultaneously running submissions for a direct clinical competitor in the same therapeutic area. This is table stakes.
- Continuity clause on health authority meetings. If the leader has represented the sponsor in a Type B or scientific advice meeting, they cannot exit the engagement without a documented transition to a named successor.
- Board reporting standard. The fractional leader owns a monthly regulatory ops status memo to the board or a designated committee. This forces the writing discipline that keeps the plan honest.
Decision Framework: Full-Time or Fractional
Use this framework as a first-pass filter. It will not make the decision for you, but it will surface whether your instinct to hire full time is a workload-driven decision or a cultural comfort decision.

The industry data supports the workload-first logic. PerfectDossier’s guidance for eCTD platform investment is that companies with fewer than approximately ten eCTD submissions per year should outsource, while those with more than 25 filings can justify buying software.6 The same threshold logic applies to a full-time regulatory ops leader: fewer than roughly 15 sequences per year and the workload does not sustain a full FTE at the senior level.
Three Case-Study Composites
The following are composites, not attributions. They represent patterns Sakara Digital and its network have observed across mid-cap biotech engagements. Details have been altered.
Case A: Series C oncology biotech, first NDA. A Boston-based single-asset oncology biotech with a lead solid-tumour compound approaching NDA filing brought in a fractional regulatory ops leader at week 32 of a 52-week filing plan. The prior structure had a director of regulatory affairs (strategy) plus a submissions coordinator (tactical) and no one owning the vendor orchestration layer. The publishing vendor was slipping timelines and the CRO was submitting draft modules straight to the sponsor without a QC step. Diagnostic surfaced eight major process gaps in the first three weeks. Fractional leader replaced the publishing vendor, put an SLA in place with the CRO, stood up a weekly integration standup, and drove the readiness assessment. NDA filed on the target date; FDA accepted for standard review without a refuse-to-file. Total fractional engagement cost through PDUFA date was $310,000 across 14 months. Estimated saving versus a permanent VP hire (base plus equity plus recruitment fee): $520,000.
Case B: Approved US product expanding to EU MAA. Mid-cap biotech with a rare disease product approved in the US 18 months earlier now preparing an MAA for the EMA. Existing US-focused regulatory ops team lacked EU procedural depth. Sponsor engaged a fractional leader with three prior EMA MAA filings for 12 months at two-and-a-half days per week. Leader owned the Module 1 EU regional adaptation, ran the pre-submission Type II scientific advice, coordinated the translation vendor for the SmPC and labelling, and managed the validation phase with EMA. Engagement cost was $215,000. The team lead retained by the sponsor became the point of continuity, and the sponsor deliberately chose not to convert to a full-time EU regulatory VP given the workload projection.
Case C: When fractional was the wrong answer. Growth-stage specialty pharma with three commercial products across five regions attempted to run a fractional regulatory ops leader for cost reasons. Within six months the model was failing: too many parallel lifecycle submissions for a two-day-per-week cadence to absorb, health authority correspondence backing up, and vendor decisions escalating to the CMO because the fractional leader was not in the room fast enough. Sponsor converted to a full-time VP by month nine. Lesson: fractional is not a cost-saving substitute for a full-time role when the workload is sustained and high-volume. Diagnose the workload first, price the model second.
Ownership Boundaries and the Sponsor Accountability Line
The single most important sentence in any fractional regulatory operations agreement is this one: the sponsor holds the marketing authorisation, and the sponsor is the responsible party in the eyes of the health authority. That does not change because someone is fractional. The fractional leader is an agent of the sponsor, not a substitute for the sponsor’s accountability. Draw that line explicitly in every engagement.
Ownership boundaries that we recommend making explicit in the engagement documentation:
- Regulatory strategy decisions (indication scope, endpoint selection, filing region selection, orphan or fast-track designations) remain with the sponsor’s head of regulatory affairs, chief medical officer, or executive committee. The fractional leader operationalises those decisions; they do not make them.
- Signing authority for submissions to health authorities remains with the sponsor’s authorised representative. The fractional leader manages the process leading up to the signature. They should not, and typically cannot, sign for the sponsor.
- Contractual authority to bind vendors remains with the sponsor’s procurement or legal function. The fractional leader recommends vendor selection and negotiates scope, but the master services agreement is signed by the sponsor.
- Post-approval commitments and pharmacovigilance responsibilities remain with the sponsor’s qualified person for pharmacovigilance (in EU) or equivalent US role. The fractional leader may coordinate the operational execution of a commitment, but the accountability is the sponsor’s.
- Data integrity and record retention obligations sit with the sponsor. The fractional leader ensures that vendor arrangements are compliant, but the records live in the sponsor’s RIM and quality systems.
When these boundaries are clearly documented, the model works cleanly. When they are ambiguous, the fractional leader ends up drifting into strategy or being asked to sign for the sponsor in ways that create real regulatory and liability exposure. The sponsor should never be able to say “we thought our fractional leader was handling that” about a health authority commitment. The fractional leader should never be signing correspondence that binds the sponsor without the sponsor’s explicit written delegation for that specific instance.
The signature question. If a fractional leader is regularly signing correspondence to FDA, EMA, or another health authority on the sponsor’s behalf, either the delegation is documented and appropriate, or the sponsor is de facto operating without an accountable regulatory head. Both scenarios need a deliberate answer, not an accidental one.
The soft handoff and evergreen documentation problem
Ownership boundaries also drive one of the most under-appreciated risks in a fractional engagement: the exit. Because the fractional leader is not accumulating tacit knowledge in an internal team member’s head, they must build the sponsor an evergreen operational documentation set from day one. The submission plan, vendor SLAs, HA correspondence conventions, RIM configuration decisions, readiness assessment templates, and cross-regional module 1 patterns need to live in a sponsor-owned wiki or SharePoint that a successor (fractional or full time) can pick up cold. We recommend a written operational handbook as a contractual deliverable at the end of the diagnostic phase, updated at least quarterly. Sponsors that skip this deliverable end up dependent on a single individual, which is exactly the fragility fractional was supposed to avoid.
Conclusion
The fractional regulatory ops model is not a new idea; it is the mature application of a workforce pattern that has already reshaped finance, medical affairs, and marketing leadership in life sciences. What has changed is that the regulatory execution ecosystem, CROs, publishing vendors, and RIM platforms, has matured to the point where a single senior leader can effectively orchestrate a global submission at two to three days per week. For most mid-cap biotechs from Series C through the first three years of commercial operations, that model matches the workload better and manages the risk more cost-effectively than a full-time VP hire made too early.
The decision is not fractional or full-time as a matter of principle. It is fractional through the workload profile that fits fractional, and full-time when the workload actually sustains it. The organisations that do this well diagnose the workload first, define the ownership boundaries explicitly, and let the model evolve as the pipeline evolves. They also plan the transition explicitly, so that if a second commercial product or a fifth region turns fractional into an under-scoped model, the sponsor has already thought about what conversion looks like and has the operational documentation to make it clean.
